Transition Coordinators, LLC started in Montgomery County, Pennsylvania, and we proudly advocate for families in all counties of the state. We connect community resources to help guide students toward their transition goals in post-secondary education, employment and vocational training, independent living, and beyond. We are committed to planning, guiding, and encouraging each student to thrive and grow into the best version of themselves!

In our private practice, we prioritize personalized support and achieving the best outcomes for families, free from the limitations set by insurance networks. While we don’t participate directly with insurances, we’re happy to provide documentation for you to submit for potential reimbursement, upon request.
