Support families in understanding their options
Assist in coordinating supports across systems
Promote informed decision-making
Transition Coordinators does not provide clinical or therapeutic services. Our work focuses on coordination, guidance, and connection to qualified providers and resources.
Our Services
Transition Coordinators provides guidance and Supports Coordination in areas such as:
Identifying and connecting families with qualified community providers, including ABA and IBHS services
Assisting with Medicaid and waiver application processes
Coordinating services across education, behavioral health, and disability systems
Supporting families during IEP meetings and transition planning
Resource navigation to help families understand available supports and options
We work collaboratively with families, schools, and service providers to support access to appropriate services based on individual needs and available resources.
