Founded in Montgomery County, Pennsylvania, Transition Coordinators, supports families across counties throughout the state by helping them navigate education, behavioral health, and disability systems.
We assist families of neurodiverse students in understanding available services, identifying appropriate resources, and coordinating supports that align with transition planning across post-secondary education, employment, vocational training, independent living, and community participation.
